SmartSense 500 w/Gen RX

Reliable, basic protection with some of our lowest monthly rates. Choose the annual deductible/monthly rate combination that works for you. Immediate benefits for the first three visits to a doctor. Your choice of prescription drug benefits, Comprehensive RX or Generic RX. Health plan that pays up to $7,000,000 per member in lifetime benefits.

Anthem Blue Cross Share 5000

This plan offers comprehensive health care services with significant in-network savings and the freedom to choose doctors and specialists out of network, with shared risk for lower premiums. You pay no deductible for office visits, covered preventive care or generic drugs.

Anthem Blue Cross Basic PPO 2500

This basic plan offers the security of in-hospital and surgical coverage to protect you against financial losses due to unexpected illness or injury. Note, to keep cost down, this plan does not include maternity benefits and coverage for professional services is limited.

Anthem Blue Cross RightPlan PPO 40-Generic Rx

This plan allows you to pay just a $10 copay for generic drugs from our Generic Rx Formulary. It also provides access to over 50,000 California network doctors and specialists and over 400 hospitals – so you’re covered just about anywhere. Significant savings for you – because we’ve negotiated lower fees with our network doctors and hospitals, your share of costs is less while paying your coinsurance. You'll also get out-of-state coverage that allows you to use your plan’s benefits when traveling.

Anthem Blue Cross PPO 3500 (HSA-Compatible)

This plan provides 100% coverage for most services after your deductible is met, with highly affordable monthly premiums, and qualifies as a high deductible plan if you wish to set up a Health Savings Account (HSA). Note it does not include maternity coverage.

Anthem Blue Cross Select HMO

This plan combines maximum savings with maximum healthcare coverage through the Select HMO network. Featuring low premiums and out-of-pocket costs, this plan offers no deductible and unlimited lifetime benefits for covered services, maternity benefits, and just a $25 copayment for office visits. Available in 22 California counties.

Tonik: Calculated Risk Taker (Anthem Blue Cross 1500)

This plan provides great all-around coverage—preventive, emergency, prescription, eyes and teeth (but no maternity coverage)—with affordable premiums and a choice of three different copay and deductible combinations to suit your lifestyle, risk factors and budget.

Anthem Blue Cross Individual Lumenos HSA 5000

This plan offers you a Health Savings Account compatible plan to work alongside an HSA that can be funded by your own pre-tax contributions. It also includes a Traditional Health Coverage component-similar to a typical health plan-which helps protect you against further health expenses.

Lumenos HSA 1500 (Without Maternity Coverage)

This plan offers you a Health Savings Account compatible plan to work alongside an HSA that can be funded by your own pre-tax contributions. It also includes a Traditional Health Coverage component-similar to a typical health plan-which helps protect you against further health expenses.
